Login to your account to upload the required forms for registration.

When in your account, select the pencil icon next to your child's name as shown here:

In your child's account, scroll down the page until the form titles are visible.  Upload each form by pressing the blue circle with the "+" next to each form as shown here:




Be sure to press the SAVE button at the bottom, and you're done.
